On 30/09/2013, an unmanned "ghost train" consisting of cars 3177+3178+2697+2698, began an uncontrolled movement out of the yard onto the southbound main track of CTA’s Blue Line. Two of the four cars had water in their control cables, which produced errant control signals to their power systems. These errant control signals instructed the cars' power systems to apply power, move, and stop, and allowed the defective cars to overcome the resistance of the two cars that were coupled to them, causing all four cars to move.
The train passed five train stop mechanisms (or automatic trip stops) between Forest Park Interlocking and Harlem-Congress Station, which were set in the stop position and each one initiated an emergency brake of the train. Following every brake application, the unoccupied cars came to a stop, but they recovered and continued to move until they struck a stopped train at the Harlem-Congress Station, consisting of units 3171+3172+3122+3121+3129+3130+3029+3030. The passenger train's leading car #3171 sustained little damage, while the ghost train's leading car #3177 sustained serious damage. There were no fatalities and 34 people were injured. Most injuries were not severe, as the collision occured at a relatively low speed.
Cars #3 and #4 of the train that caused the accident, that is, #2697 and #2698, were not working properly and were being stored until they could be moved to a facility for troubleshooting and repair. During the night, the cars were powered so their interiors could be cleaned by the night cleaning crew which appearently left them powered on after finishing their task. Two CTA employees were fired and other two were suspended following this accident.

Renumbered 3458.
Ex train 3031+3032. 3031 was damaged in a derailment at Wilson on March 15, 1988 when a northbound North-South Route train it was a part of split a switch, and 3032 was renumbered 3458 afterwards.
It is coupled with the 3200-series #3457, the last of the 3200-series.

Last car of the 3200 Series. It had no pair of its type, as it was ordered as replacement pair for the 2600-Series car 3032, which was renumbered 3458.

Ex 3032 -- Budd CTA 2600 series
There were only 256+1 cars of this type (Morrison-Knudsen 3200 series). An additional car was added to the order -- bringing the total to 257 cars -- which was paired with 2600-series car 3032, whose mate was damaged in an accident. Car 3032 was renumbered 3458 on February 25, 1994, and paired with car 3457, created the first "mis-mate" that consisted of two cars of different series.
